Per BB 9313 "Exhibit pages are intended to be easily updated. They are provided as information items to Board members but are not subject to approval."

APC was removed from the title in Exhibit 0420(a) Site Council/PAC/Governance Year End Report, as Charter Schools will replace this form with Exhibit 6187(c) Charter School Annual Review Form to follow the APC reporting. 

Thus, all traditional schools will complete Exhibit 0420(a) Site Council/PAC/Governance Year End Report and all Charter Schools will complete Exhibit 6187(c) Charter School Annual Review Form.

The compilation of the Site Council End of Year Reports will still be included in the General Information Packet at the end of the school year, and now a compilation of the Charter School Annual Review Forms will also be included in this General Information Packet. 

Exhibit 6187(c) Charter School Annual Review Form is also provided for reference.
