Middle School Handbook Changes

1. Page 10, IV. STUDENT ELIGIBILITY, Section A, Parent/Student Responsibilities for Participation, Item 3 and Item 4. Procedural change to include Concussion Information and Sudden Cardiac Arrest for middle school parents and students. (Obtained from ASAA, see enclosed documents)
2. Concussion Information Parent and Student Verification. Student acknowledgement they have received a copy of “Head’s Up: Concussion in High School Sports – A Fact Sheet for Athletes and parent acknowledgement they have received a copy of “A Parent’s Guide to Concussions in Sports” and understand its contents.
3. Sudden Cardiac Arrest. Parent and student acknowledge they reviewed and understand the symptoms and warning signs of SCA.
